This study half-day on Medicines Management is specifically for registered nurses working in Nursing Homes in South Gloucestershire. It will provide an opportunity to upskill these staff through meeting the following objectives:

· Demonstrate improved medicines management practices in the care home

· Explain the medicines management skills required for a safe and legal practice

· Provide underpinning knowledge to facilitate the administration of medication

· Discuss best practices, new policies and procedures

· Provide an opportunity for continuing professional development

The session will be delivered by Kassin Yakhlef, Care Home Pharmacy Technician with NHS Bristol, North Somerset & South Gloucestershire CCG.
It is FREE to the identified staff working in Nursing Homes.

Please note:
Certificates of attendance will be issued at the end of the session; this will contribute towards three hours of CPD requirements for nurses’ revalidation, or give the opportunity to enhance the knowledge and competence of senior carers involved in the administration of medicines.

Learning outcomes
By the end of this session for Registered Nurses only, focusing on Medicines Reconciliation and Administration of Medicines via Feeding Tubes, delegates will be able to:

Medicines reconciliation: 

¦ Define medicines reconciliation

¦ Explain why medicines reconciliation is important

¦ Demonstrate how to apply NICE guidelines and CQC standards to medicines reconciliation

¦ Explain why the transfer of information between care settings is so important

¦ Determine if a drug history is not accurate

¦ Identify the type of errors and discrepancies during the transfer of care 

 
Administration of medicines via feeding tubes:

¦ Identify the legal and financial implications of administering medication via feeding tubes

¦ Explain the factors which should be considered before giving a medicine via a feeding tube

¦ Demonstrate how to safely administer medication via feeding tube

¦ Understand the role of the pharmacy team in supporting the safe administration of medicines

¦ Evaluate when to review this practice
