Half-day workshop delivered by Central Training & Operational Staff.
Workshop Aim: To share and reflect with colleagues on best practice use of re-ablement to promote wellbeing
Learning outcomes
By the end of the session delegates will be able to:
• Describe the purpose and value of a reablement service to improve wellbeing for a range of service users
• Explain ways to identify personal outcomes with service users
• Draw on best practice to write specific outcomes and complete a PI plan.
• Explain the reablement process in South Gloucestershire in the new structure of cluster working
